Imodium

What is Imodium and how does Imodium work?

Imodium is a prescription only drug manufactured by Ortho-McNeil-Janssen Pharmaceuticals, Inc. which belongs to the class of drugs called Antidiarrheals. It was first approved by the FDA in 1976. Imodium is not a controlled drug with a pregnancy category C. Imodium is a brand name of the generic Loperamide. Other brand names of Loperamide Diamode, Imodium A-D, Imodium A-D EZ Chews, Imodium A-D New Formula, Kao-Paverin, Kaopectate 1-D. Imodium is the most commonly available and used among all of these drugs.
Imodium is mainly sued to slow down the rhythm of digestion so that the small intestines have more time to extract nutrients from the food that is present in the stomach. It is used to treat diarrhea and also reduce the amount of stool in people who have an ileostomy where the bowel is re-routed through surgery via an opening in the stomach.
Imodium generic is called Loperamide which is also known as the systematic name 4-[4-(4-Chlorophenyl)-4-hydroxypiperidin-1-yl]-N,N-dimethyl-2,2-diphenylbutanamide. The protein binding of generic Imodium os 97% and the biological half-life is 9-14hours. It is excreted in feces in the range of 30-40% and the approx 1% is excreted through the urine.
It is often used for the treatment of gastroenteritis, inflammatory bowel disease and short bowel syndrome. It is also effective to control and decrease the frequency of diarrhea. The drug works by slowing down the contractions of intestines and helping them to process the food for a little longer. The drug is effective for the treatment of a number of types of diarrhea which include acute nonspecific diarrhea, mild traveler’s diarrhea, chronic diarrhea caused by bowel resection and chronic diarrhea secondary to inflammatory bowel disease.
The generic Imodium is part of the WHO’s list of essential medicines in a basic health system. The drug is not very expensive and it is available in the range of 0.004 o 0.04 per dose wholesale whereas in the US, it’s priced at about 0.20 dollar a dose.

Imodium dosage

Imodium is a prescription only drug as per the FDA rules and it should only be taken after consultation with a doctor.
1 capsule of Imodium has a strength of 2mg. The recommended initial dose for adults for the treatment of acute diarrhea is 4mg followed by 2mg after each missed stool. The daily dose is a maximum of 16mg. You may not notice in an improvement of diarrhea right away since it takes up to 48hours to show clinical improvement at the recommended Imodium dosage.
Imodium for kids has a different dosage. In children of 2 to 5 years, the liquid formulation (IMODIUM® A-D 1 mg/7.5 mL) should be used for best effect. Older children between ages 6 to 12 can either use IMODIUM capsules or IMODIUM liquid can be used as per choice, liking or ease.
In the case of Chronic Diarrhea, the recommended initial dose is 4mg followed by 2 mg after each unformed stool until condition improves. The dosage can change thereafter upon monitoring the condition and effectiveness of the first dose.
Imodium dose is not recommended to be used on children under the age of 2.

Patients suffering from hepatic impairment due to first pass metabolism
Patients with HIV
Cases of symptomatic C
Children under the age of 2. Some countries such as Pakistan have banned the use of antidiarrheal loperamide
Pregnant women or breastfeeding mothers
Some of the commonly occurring side effects of using Imodium for diarrhea treatment are:
Constipation
Abdominal cramps or pain
Dizziness
Nausea
Some rare but more severe side effects are listed below:
Paralytic necrolysis
Urinary retention
Heat stroke
Stevens-Johnson Syndrome
Toxic megacolon
Angioedema
If someone takes an overdose of Imodium, some symptoms to be noted are:
Vomiting
Pain in stomach
Burning sensation
Vomiting and drowsiness

Most people who use this drug will likely not face any of these side effects, but that is not a reason to not be aware of the potential risk involved.
